We focus on chronic diseases such as diabetes, heart disease, and hypertension, which disproportionately impact Black and Latine residents at higher rates. We promote chronic disease prevention through community engaged and community driven interventions that focus on nutrition, physical activity, and improving connections between health care and the community. Much of this work is supported through a grant from the Centers for Disease Control and Prevention (CDC) Racial & Ethnic Approaches to Community Health (REACH) program.
We also collaborate closely with the Yale-Griffin Prevention Research Center to develop, implement, and evaluate research-driven, evidence-based practices that can improve the health inequities we see in New Haven today.
