Our Programs & Projects

Health Leadership Programs

We have several health leadership training programs for New Haven residents including New Haven Health Leadership program, Community Research Fellowship, and the Vaccine Equity Fellowship:

  • New Haven Health Leaders (NHHL). The New Haven Health Leaders put their training into practice by focusing on neighborhood-level health issues, while engaging other residents and stakeholders towards the mission of improving health for their communities.

  • Community Research Fellows (CRF). This program is designed to create a network of residents with knowledge, skills, and confidence for a successful partnership with researchers.

  • Vaccine Equity Fellows (VEF). The Vaccine Equity Fellows will activate their neighborhood or community-based networks to conduct vaccine outreach as trusted messengers.


REACH

The Racial and Ethnic Approaches to Community Health (REACH) program is one of the only CDC programs that explicitly focuses on reducing chronic diseases for specific racial and ethnic groups in urban, rural, and tribal communities with high disease burden.

In New Haven we are focused on reducing health disparities among Black and Latinx residents, with a particular focus on low-income neighborhoods. We do this through culturally tailored interventions to address nutrition, physical activity, and links between clinical partners and the community.


Yale-Griffin Prevention Research Center

The Yale-Griffin Prevention Research Center (Y-G PRC) works to identify public health problems and to develop, test, and evaluate public health interventions that can be applied widely, particularly in underserved communities. Established in 1998, the Y-G PRC is co-housed at the Yale School of Public Health in New Haven and Griffin Hospital in Derby. Y-G PRC is one of 25 CDC-funded Prevention Research Centers around the country, with its focus on chronic disease prevention. 

CARE works hand in hand with the Y-G PRC on multiple projects focused in New Haven.
